Welcome to Student Health Services
The Miami University Student Health Service provides outpatient care to all eligible students. Eligible students include anyone from the Oxford or regional campuses who are enrolled in at least one class.
Our staff includes full-time physicians, certified nurse practitioners and a nursing staff to provide quality outpatient care to the students of Miami University. Our services include general medicine and injury care, gynecology, immunizations, laboratory and pharmacy. All of the SHS services must be ordered by a SHS clinician. Our goal is to provide both medical care and education that supports a healthy college experience.
The SHS is opening with a new scheule for the 2013-2014 school year. Begininning Monday, August 26, our hours will be the following:
- Academic year - Monday and Wednesday 8 a.m. to 7 p.m., Tuesday, Thursday and Friday 8 a.m. to 5 p.m., Saturday 10 a.m. - 2 p.m. (limited services)
- Summer hours - 7:30 a.m. to 4:30 p.m.; closed 12 -1 p.m. for lunch
- Academic breaks - 8 a.m. to 5 p.m.

Online appointment option: Illness and injury appointments can also be made on-line via the Student Health Services web portal. Please access this service via the link below, and follow the directions. Special department appointments - physical therapy, immunizations, physicals and gynecology must be made by phone.
Bring your Miami ID card and your insurance card to your appointment.
Flu Shots are available. Please call 513-529-3000 to make an appointment. Cost is $25.
Medical Form Web Portal
All new Miami University students - first year, graduate, or transfer - can submit the medical form via the web portal link below. Please read the directions first.
Directions: You must register in the system with your unique ID, date of birth and your Miami e-mail address. A new password will be sent to your e-mail account. Your unique I.D. and this new password is used to log in to the web portal. You will be submitting a personal history, dates of your vaccines and a family history. If you experience any difficulties accessing the medical form on the web portal, please call 513-529-3000.
Deadline for completion is Sunday, Sept. 15, 2013, for the 2013-14 school year.

All full time Oxford students are required to complete this waiver each school year if already covered by health insurance.
The electonic waiver is not active. (Submitting this electronic waiver will exempt you from the health insurance policy that the university offers. You will be asked to provide information about your coverage so have your insurance card ready.)

The GARDASIL vaccine designed to protect againist the four most aggressive types of human papillomavirus (HPV) - the cause of genital warts & cervical cancer - is now available at the Student Health Service.
