Submitting Applications for Review

Prior to attempting to complete an application, you will need to complete training requirements.
Refer to the MU guidelines and other references listed below and provided with the application packet for details on completing the application.


Each application packet (zip file) contains three files: there is a coverpage template, a research description template, and a guidance document that provides information on how to address the questions posed on the application. The cover pages and research description are submitted to the IRB. Each are in a generic word-processing format and must be completed using a computer and submitted electronically.

Students cannot submit applications directly to the IRB. The application is first sent to their faculty advisor and the faculty advisor submits the application for review. Information regarding this policy can be found at these web pages: Policy, FAQ.

When completed, submit your application as two files using your MU email account:

  • File 1: The completed cover-page;
  • File 2: The research description with all attachments (e.g. recruitment flyers, consent form, surveys, etc). File 2 can be sent as either an MS-Word or PDF type document
  • Please include your name in the filenames and it is helpful if the files are numbered in the correct order, e.g. 1_smith_coverpage.docx
  • Faculty: attach the files to an email message and send to humansubjects@MiamiOH.edu
  • No signatures are required if using your MU email account.

The federal regulations (45 CFR 46) governing research involving human subjects describe some categories of research that can be exempted from IRB review. Additionally some research projects do not meet the regulatory definition of research, thus do not require IRB review.  Click below for more information
