W-2
W-2's are available on BannerWeb. Once you have logged on select Employee Services Online tab. You will be able to view and print multiple copies of your W-2. Contact Payroll Services at 529-6223 with any questions.
How do I change my personal information?
Changes such as your home address, telephone, and emergency contact may be made online, using BannerWeb. Once you have logged on choose the Personal Information tab. However, if you need to change your name you must provide legal documentation and come to the Academic Personnel Services Office, Room 1, Roudebush Hall.
How do I change my work address?
Changes such as your K1 Work address and work telephone number may be made online, using BannerWeb. Once you've logged on choose the Employee Services Online tab. Click on "Update Campus Work Address" and from here you can add, edit or delete an address and telephone number.
Computer Loan Program

Annual appointments
In early August, continuing faculty receive an annual letter of appointment which details salary and status for the upcoming year. There is no contract to sign; it is assumed that the faculty member accepts the appointment as stated unless Academic Personnel Services is notified to the contrary before the beginning of the academic year.
Initial appointment of temporary, full-time faculty
Temporary, full-time faculty receive the initial appointment in a letter from the Provost, which requires an acceptance.
Reappointments
Temporary, full-time faculty are reappointed through an on-line process and will receive an email notification of the reappointment. This email will direct you to On-Line Contracts to provide an e-signature acceptance of the appointment. These appointments are typically made between March and early August.
Notification of non-renewal
Per MUPIM 9.6, temporary, full-time faculty are notified by February 1st of the University's intent to not extend an offer of reappointment for the upcoming academic year. This notification is in the form of a letter and is sent to the home address. This notification does not preclude a subsequent reappointment. However, temporary full-time faculty are limited to serving five years in a full-time instructional position.
Cost share and release
If a portion of a faculty member's workload is to be covered by a grant or other external funds (bought out), a form indicating the time period, salary amount and source of funding must be completed, approved and submitted during the semester of the buyout. To access form click here.
Assigned Research Appointment
Assigned Research Appointment provides for disciplinary and pedagogical research by releasing faculty member from teaching for one semester. To view full description see MUPIM 6.7. To access form only, click here.
Faculty Improvement Leave
This program provides release from teaching duties and other University assignments, either full compensation during one semester or two-thirds compensation during two semesters, continuation of University-provided insurance benefits and fee waiver, and eligibility for salary increment and promotion. To see full description see MUPIM 6.8.
Summer Compensation
The 24% and 30% limits to summer earnings for full-time faculty are no longer in effect. Effective summer 2011, the ceiling has been raised to 33 1/3% of the prior year’s base salary (three months of base salary) for any combination of duties (teaching, workshops, research, administrative, special assignments). The formula used to determine one’s instructional salary has not changed – 3% of base salary per credit hour (unless other departmental/campus guidelines exist). For grant-based earnings, the funding agency may have income limitations.

Unclassified Staff Performance Evaluations

A performance evaluation form, either one described below or one that has been adopted in your division, must be submitted along with the Professional Development Plan to complete an unclassified staff performance evaluation.
 
A more in-depth evaluation instrument, Performance Assessment & Development Instrument, which assesses nine core values using behavior measures, is available and is especially useful for an initial evaluation.

A shorter version of the evaluation instrument, Performance Appraisal, includes a rating scale and may be used. The document, Behavior Descriptions, will assist in defining the core values.

The Professional Development Plan (PDP) form is now required and must be submitted along with an evaluation instrument.

Monthly Leave Reporting
Unclassified Staff report the use of vacation and/or sick leave on a monthly basis via the BannerWeb Employee Services Online. Vacation
The purpose of the vacation benefit is to provide time away from work for rest, recreation and renewal; the University, therefore, expects its unclassified administrative staff to use vacation time each year for such purposes. It is also recognized, however, that it may not always be possible for an individual to use his or her full complement of vacation days within a given contract year; for this reason, while accumulation of vacation time is discouraged, it is permitted within the limits set forth below. All vacation must be approved in advance by the staff member's supervisor. See MUPIM 4.2 for specifics regarding Miami's vacation policy.
Miami P.R.I.D.E. Award
An Award Program created for classified and unclassified staff to recognize and reward significant outstanding performance. To review the guidelines click here. Initial Appointment
Academic Personnel Services will send a Welcome letter with Terms of Employment which will guide you through the process of completing a New Hire packet at Academic Personnel Services.

These appointments are typically made between March and early August for the fall semester and October and January for the second semester.
